Thank you for your willingness to help the students of the Middletown Unified School District! We recognize and value the volunteers who provide a variety of services to our schools. Volunteers are viewed as role models to students,  and individuals who can provide support throughout MUSD’s programs. This webpage contains information and outlines procedures to ensure that all volunteers have been screened appropriately to provide for maximum safety for all students in MUSD.

 

 Parents, students, and community members who fulfill the mandatory volunteer requirements established by the Middletown Unified School District may volunteer.  In order to determine the requirements appropriate for the level of service you wish to provide, the following definitions and requirements are explained below. 

 

All volunteers in any capacity must have all of the prerequisites completed and on file with the Human Resources Office.  Parents and/or guardians are not prohibited from being active in their children(s) education; however, they may be limited in volunteer capacities due to prior convictions and/or probationary periods.

STEPS TO REGISTRATION AS AN APPROVED VOLUNTEER

 

  1. Complete a Volunteer Data Form. The form is valid for the current school year only.

  • Copy of a valid Tuberculosis Test (TB test) 
  • Copy of completed Live Scan Document – Only if this is your first time volunteering in our district
    • Please have the following documents readily available when completing the form online. You will be asked to upload all documents.

  1. If wanting to volunteer to drive continue below. If not wanting to drive Human Resources will contact you as soon as your fingerprints have been cleared.

DRIVING & TRANSPORTING STUDENTS

 

 Individuals transporting students in a personal or district vehicle must register on an annual basis with the District:

Complete Driver Registration Form.

Please have the following documents readily available when completing the form online. You will be asked to upload all documents.

 

This form must be completed annually in order to be considered eligible to drive students in a personal and/or district vehicle.
